What would you do if there were a CD incident in the pharmacy? Supposing a locum were working that day? Do you have the necessary information to deal with an incident? Where is it kept? Is it up-to-date ?

Concise Investigation Report template
All relevant agencies will be informed by CSU, who may contact you for further information.
Make sure your CD register accurately records the drugs involved; you have prescriptions to cover re-issues etc. It might make sense to keep the incident report + follow-up and photocopies of supporting documentation – register page(s), Rxs, notes of conversations etc.
Review and update (and possibly) strengthen all relevant SOPs.
